Sunken manhole with uneven pavement

There is a sunken manhole that has now caused the path to become uneven and a pothole developed. Someone has this morning had a fall and sustained a head injury after getting a pram wheel stuck in the hole. This needs to be looked at asap as this is a public safety risk.
