Multiple pot holes, crumbling verges and ruts caused by hgv's

This whole road is unsafe. there are dozens of pot hole, the drain at the top of the road floods the road and causes a stream to travel down the road. the road has big ruts in places, caused by lorries - in the winter, this caused my car to skid and i ended up in a ditch, with a repair costing over £600.
Although a back road, this is the main route for several businesses and the state of this road is awful - I actually dread using it every day.
Someone needs to come out and view this whole stretch and do an assessment on it, as the issues raised on the map do not show the full extent of the issues.
