Drain cover sinking, been 'repaired' twice but keeps coming back. It makes a loud banging when cars drive over it and it's disrupting sleep and is affecting getting on my driveway.

The drain cover is collapsing in the middle of the road, this was supposedly fixed by pouring concrete on top. This has caused the drain to seperate further creating an even larger pot hole in the road. The drain cover has continued to sink and remains loud as drivers drive over this. The noise is keeping myself and neighbours awake at night. Those with young children are also being disturbed. This is also causing issues in pulling onto out drive way. Please fix this mess.
