A Slabs - Uneven / Damaged / Cracked

Sent to North Northamptonshire Council less than a minute later. ref: 6086666.
The path running down west glebe road from St Andrews church to the allotments on the left hand side is in such bad shape that I have been thrown out of my wheelchair 3 times this week. My wheels either get stuck in the cracks and holes or my foot rest get caught on the mounds caused by the tree roots pushing the tarmac up. Not only has this damaged my chair but it causes me pain as you can imagine. I am fed up of being trapped in my home because I cant get down the path without the risk of injury or damage.
